Who is Served?
This is the only Daily Bread program where people have to sign up to receive food. But it little invades one’s privacy. The USDA requires completion of a simple, one-page form, which will tell USDA how much food we can pick up from the warehouse. Click here to review or print the application form.
If you plan to have someone else pick up your food, please use a proxy form, signed by both you and the person you are authorizing to pick up your food.
To request service, contact a representative of Radford-Fairlawn Daily Bread, USDA, or Women's Resource Center. The representative will fill out an application form for you. The completed application may be
- turned in at Radford-Fairlawn Daily Bread at 501 East Main St. on the 3rd Friday of the month between 11 a.m. and 12:15 noon or

Once a month, volunteers pick up food at the Second Harvest Food Bank in Salem and take it to the Radford 501 East Main Street dining room, where it is sorted into individual allotments to be picked up by registered recipients on the third Friday of each month.
